You are going to need to either instrument the app itself or install something like New Relic in order to get visibility into where the time is going on the back-end. From the outside it's a complete black box.

I HIGHLY recommend giving New Relic a try. It's free for trying out and you can usually get it up and running in just a few minutes. It will point you to your bottlenecks almost immediately.
